Task Summary: The Medication Reconciliation Professional plays an essential function in assisting to avoid client medication mistakes triggered by incorrect or insufficient info in the Emergency situation Department. Under the guidance of a pharmacist, and according to department policies, treatments and standards, the Medication Reconciliation Specialist will help pharmacists in acquiring a precise medication history by speaking with clients and talking to their outpatient drug stores.

Important Obligations:

  • Gets medication histories on all appointed clients throughout set up shifts.
  • Carries out client and/or caretaker interviews to acquire dosage and frequency of existing medications consisting of prescriptions, non-prescription medications and organic supplements.
  • Calls outpatient drug stores or other health care centers to validate medication programs or examine disparities when needed.
  • Interacts medication associated problems to all proper heath care experts.
  • Preserves Emergency situation Department ADM’s consisting of restocking, expiration date monitoring and basic tidiness. Will regularly evaluate stock equipped in the ADM’s to guarantee proper medications exist.

Needed Certifications: High School diploma or GED needed.

  • License Registered Drug store Tech required., and Certificate 1 Pharm Tech Cert Board needed.
  • 3-5 years associated work experience needed.
  • Strong working understanding of brand name and generic drug names and standard pharmacology.
  • Standard familiarity with computer systems. Capability to browse at a fundamental level within web-based applications.
