Biden's massive infrastructure plan, a Covid vaccine for teens and the Final Four is set

President Joe Biden is ready to unveil his $2 trillion dollar infrastructure plan, plus more good news on Covid-19 vaccines and with another upset, the men’s Final Four is set.

Here’s what we’re watching this Wednesday morning.


‘The wish list’: Biden makes his move with push for far-reaching infrastructure overhaul

In a speech Wednesday, President Joe Biden will lay out the first part of a massive, multitrillion-dollar infrastructure plan that is expected to include projects as varied as highways and “human infrastructure,” like child care.

The kitchen-sink approach is designed to push the economy in a greener and more equitable direction, paid for with higher taxes on corporations and the wealthy, NBC News’ Alex Seitz-Wald and Shannon Pettypiece report.

The $2 trillion proposal, which is being called the American Jobs Plan, will focus on projects that could get bipartisan support, like roads and bridges, expanded broadband internet access and investments in clean energy. It also includes an expansion of home health care services, which are in demand for the aging population.

The sweeping legislative package would be legacy-making for the nascent Biden administration, but it will face a steep climb up Capitol Hill, where time to pass major bills is already waning.

“There is a sense of spending fatigue on some quarters on Capitol Hill, not only among Republicans but some Democrats, as well,” said one Democratic political strategist.

Others might argue that now is the time to act and spend his political capital: Biden currently has high national approval ratings on his handling of the economy and Covid-19, according to new polling. But, there are warning signs, too: He’s getting lower marks on culturally divisive issues like immigration and gun control.

Pfizer says its Covid vaccine is 100 percent effective in young teens

There is positive news on the vaccine front: the Pfizer and Moderna Covid-19 vaccines have been found to be 90 percent effective in their “real world” study, according to the Centers for Disease Control and Prevention. COVID Vaccines Could Lose Their Punch Within a Year, Experts Say

Two-thirds of epidemiologists from leading academic institutions say the world will need new or modified vaccines for COVID-19 within a year, new research shows.

In a survey of 77 epidemiologists from 28 countries by the People’s Vaccine Alliance, 66.2% predicted that the world has a year or less before variants make current vaccines ineffective. The People’s Vaccine Alliance is a coalition of more than 50 organizations including African Alliance, Oxfam, Public Citizen, and UNAIDS (the Joint United Nations Programme on HIV/AIDS).

Almost a third (32.5%) of those surveyed said ineffectiveness would happen in 9 months or less; 18.2% said 6 months or less.

Paul A. Offit, MD, director of the Vaccine Education Center, Children’s Hospital of Philadelphia in Pennsylvania, told Medscape Medical News that while it’s hard to say whether vaccines could become ineffective in that timeframe, “It’s perfectly reasonable to think it could happen.”

The good news, said Offit, who was not involved with the survey, is that SARS-CoV-2 mutates slowly compared with other viruses such as influenza.

“To date,” he said, “the mutations that have occurred are not far enough away from the immunity induced by your natural infection or immunization such that one isn’t protected at least against severe and critical disease.”

That’s the goal of vaccines, he noted: “to keep people from suffering mightily.”

A Line May Be Crossed

“And so far that’s happening, even with the variants,” Offit said. “That line has not been crossed. But I think we should assume that it might be.”

Offit said it will be critical to monitor anyone who gets hospitalized who is known to have been infected or fully vaccinated. Then countries need to get really good at sequencing those viruses, he added.

The great majority of those surveyed (88%) said that persistently low vaccine coverage in many countries would make it more likely that vaccine-resistant mutations will appear.

Coverage comparisons between countries are stark.

Many Countries Haven’t Given a Single Vaccine Dose

While rich countries are giving COVID-19 vaccinations at the rate of a person a second, many of the poorest countries have given hardly any vaccines, the People’s Vaccine Alliance says.

Additionally, according to researchers at Duke University’s Global Health Innovation Center, high- and upper-middle-income countries, which represent one-fifth of the world’s population, have bought about 6 billion doses. But low- and lower-middle-income countries, which make up four-fifths of the population, have bought only about 2.6 billion, an article in Nature reports.

“You’re only as strong as your weakest country,” Offit said. “If we haven’t learned that what happens in other countries can (affect the global population), we haven’t been paying attention.”

Gregg Gonsalves, PhD, associate professor of epidemiology at Yale School of Public Health, Yale University, New Haven, Connecticut, one of the academic centers surveyed, didn’t specify a timeline for when vaccines would become ineffective, but said in a press release that the urgency for widespread global vaccination is real.

“Unless we vaccinate the world,” he said, “we leave the playing field open to more and more mutations, which could churn out variants that could evade our current vaccines and require booster shots to deal with them.”

“Dire, but Not Surprising”

Panagis Galiatsatos, MD, MHS, a Johns Hopkins pulmonologist whose research focuses on healthcare disparities, said the survey findings were “dire, but not surprising.”

Johns Hopkins School of Medicine in Baltimore, Maryland, was another of the centers surveyed, but Galiatsatos wasn’t personally involved with the survey.

COVID-19, Galiatsatos points out, has laid bare disparities, both in who gets the vaccine and who’s involved in trials to develop the vaccines.

“It’s morally concerning and an ethical reckoning,” he told Medscape Medical News.

Recognition of the borderless swath of destruction the virus is exacting is critical, he said.

The United States “has to realize this can’t be a US-centric issue,” he said. “We’re going to be back to the beginning if we don’t make sure that every country is doing well. We haven’t seen that level of uniform approach.”

He noted that scientists have always known that viruses mutate, but now the race is on to find the parts of SARS-CoV-2 that don’t mutate as much.

“My suspicion is we’ll probably need boosters instead of a whole different vaccine,” Galiatsatos said.

Among the strategies sought by the People’s Vaccine Alliance is for all pharmaceutical companies working on COVID-19 vaccines to openly share technology and intellectual property through the World Health Organization COVID-19 Technology Access Pool, to speed production and rollout of vaccines to all countries.

In the survey, 74% said that open sharing of technology and intellectual property could boost global vaccine coverage; 23% said maybe and 3% said it wouldn’t help.

The survey was carried out between February 17 and March 25. Respondents include epidemiologists, virologists, and infection disease specialists from the following countries: Algeria, Argentina, Australia, Belgium, Bolivia, Canada, Denmark, Ethiopia, France, Guatemala, India, Italy, Kenya, Lebanon, Norway, Philippines, Senegal, Somalia, South Africa, South Sudan, Spain, United Arab Emirates, Uganda, United Kingdom, United States, Vietnam, Zambia, and Zimbabwe.

Offit and Galiatsatos reported no relevant financial relationships.

    Covid cases are rising again. Who's getting sick?

    People under 60 are accounting for the majority of new Covid-19 cases across the country — likely a testament to the success of the vaccines that have been administered to primarily older, more vulnerable Americans.

    The number of cases is rising again following a steep decline and then plateauing for several weeks. During a White House Covid-19 briefing Monday, Centers for Disease Control and Prevention director Dr. Rochelle Walensky said the rise in cases — an average increase of 10 percent from the previous week — gave her a sense of “impending doom.”

    But doctors say that patients seeking care this time around are younger and, notably, not as sick.

    In New York, where cases are rising, some Covid-19 patients still require hospitalization, but the numbers of such patients are nowhere near the “astronomical” levels from a year ago, said Dr. Frederick Davis, an associate chair of emergency medicine at Northwell Health’s Long Island Jewish Hospital.

    Now, Davis said, Covid-19 cases are mostly mild.

    “The cases we are seeing are the younger groups that probably aren’t eligible for vaccines just yet,” Davis said.

    More than 73 percent of people over age 65 has received at least one dose of the Covid-19 vaccine, and nearly half are fully vaccinated. Monday, the CDC reported that the Pfizer-BioNTech and Moderna vaccines are 90 percent effective against Covid-19 in the real world.

    The increase in immunity among older adults is illustrated in the shift in age groups most likely to seek care for Covid-19.

    Nationwide, “the number of 25-to-49-year-olds visiting U.S. emergency departments for diagnosed Covid-19 is now higher than the number of visits among patients 65 and older,” the CDC said in a statement to NBC News.

    In Michigan, where cases have more than doubled in the past two weeks, doctors are seeing increases in patients between the ages of 10 and 60, with hospitalizations among people in their 50s rising at the fastest rate, said Dr. Joneigh Khaldun, the state’s chief medical executive.

    “Even if you may be at lower risk for getting the virus, you can still get the virus,” Khaldun said. “People simply cannot let their guard down now. No one lives in a bubble.”

    Dr. Usamah Mossallam, an emergency department physician at Henry Ford Health System in Detroit, cautioned that even younger people can get quite ill from Covid-19.

    “I’ve also seen my fair share of people who require hospitalization and ICU admissions, and they’re in their 40s,” Mossallam said.

    At Northwell health system in New York, Davis said that many of the Covid-19 cases are due to domestic travel.

    “They went to a gathering somewhere out of state, whether it was family they hadn’t seen in a long time, or maybe a wedding,” Davis said. “Sometimes multiple family members come in from the same event.”

    Spring break may also be partly to blame for the recent rise in cases, particularly as some states have rolled back restrictions and mask mandates, said Dr. Joseph Khabbaza, a pulmonary and critical care medicine specialist at the Cleveland Clinic. But vaccinations among the highest-risk demographics may prevent hospitalizations from rising as precipitously this time as they did during previous surges, he said.

    “A lot of places opened up and we saw more travel for spring break, so we certainly expected a bump in cases,” Khabbaza said. “But when these people go back home to their workplace or to their loves ones and family members, at least more of the higher risk people are going to be vaccinated.”

    The trend toward younger patients with milder illness is likely the reason many intensive care units are no longer inundated with severe cases.

    At the pandemic’s peak last year, Dr. Todd Rice, director of Vanderbilt University Medical Center’s medical intensive care unit in Nashville, Tennessee, said he had 60 Covid-19 patients at once. Now, he has five.

    The shift is likely because “we’ve changed the demographics of the people who are being affected” by the illness, Rice said.

    The virus itself, however, is shifting and changing. “We know that about 26 percent of all sequenced virus is now the B.1.1.7” variant, Walensky said during Monday’s briefing. That variant has been shown to be more contagious.

    “This variant is probably less forgiving, and more infections will occur,” Walensky said.

    Khabbaza said that while he remains hopeful that a new wave of infections won’t be accompanied by as many hospitalizations and deaths, it’s hard to predict the full impact of the variants.

    “We haven’t had a surge with a variant yet, so we’re not really sure what to expect for these next couple weeks and months,” he said.

    Even if cases are milder, they can still result in debilitating “long-hauler” symptoms.

    The possibility of long-term illness combined with more transmissible variants are major reasons to get vaccinated, even as cases are trending toward younger, healthier patients.

    “The good news with the variants is, they are all mostly responsive to vaccinations,” said Dr. James McDeavitt, senior vice president and dean of clinical affairs at Baylor College of Medicine in Houston.

    “We can still vaccinate our way out of this crisis.”

    Resolves discrepancies, troubleshoots and runs reports from the automated dispensing machine (ADM)

    Cleans and helps maintain equipment and work area; sterilizes glassware according to established procedures.

    KEY SUCCESS FACTORS

    Basic understanding of Pharmacy Care modalities, terminology, pharmaceutical calculations, terminology, drug use and aseptic techniques, standards, and equipment.

    Knowledge of procedures and standards for the labeling and dispensing of prescription medications.

    Must be able to read, write and follow instructions and flow chart protocols.

    Ability to sort, check, count, and verify numbers.

    Ability to follow established safety, infection control, environmental and isolation practices and procedures at all times.

    Interpersonal skills required to interact effectively with patients and staff.

    General computer skills, including but not limited to: using required software applications, data entry, information security, electronic medical documentation, hand held scanning and email.
