Medication History Advocate / Pharmacy Technician
The medication history advocate / pharmacy technician performs medication histories for new hospital admissions. Under general supervision of a clinical pharmacist, the medication history advocate obtains/updates medication histories for newly admitted patients, reviews available medication history in the electronic medical record, and documents complete and accurate information for each medication (e.g., drug name, dose, route, frequency, indication, and date/time last dose was taken). The medication history advocate uses available resources for medication history collection including, but not limited to: patient/family interview, medication lists, outside facility medication administration records, outpatient pharmacy records, and physician office records. Also, this position verifies/updates patient allergy, adverse drug reaction, preferred pharmacy and primary care physician information in the electronic medical record. Excellent communication skills and customer service are essential.
- Ability to navigate electronic health data base and enter new medication history or update existing
- Uses good clinical judgment skills to determine accuracy of medication history list as obtained – solicits other sources of information to verify accuracy.
- Collaborates with charge nurse and patient nurse to determine when patient can be interviewed.
- Has excellent knowledge of brand and generic medication names, common dosages and dosage forms and frequency of administration of each medication..
- Ability to communicate with patients of all education levels in an Emergency Department setting
- Assess if the current list of medications is complete by looking at drug, dose, frequency, indication for use.
- Establish rapport with patient to obtain medication history or determine if patient or family member can provide medication history or call patient’s pharmacy or physician’s office for medication history Determines compliance of medication consumption prior to admission
- Obtains indication and frequency of use for “as needed” medications
- Prompts patient for additional medications, by specifically suggesting over the counter (OTCs) medications including herbals, vitamins, pain relievers, cold products patches, lotions & creams, eye drops, sprays, and inhalers. Determine if patient was on recent antibiotics prior to admission
- Obtain medication allergy history and related reactions to the medication
- Assists the transition of care clinical pharmacist by ensuring patients identified for discharge counseling and medication reconciliation have a pharmacy services completed medication history.
- Prepares education materials for the transition of care clinical pharmacist as necessary.

What You Will Do
Obtain and document through the electronic medical record (EMR) both timely and accurate medication histories for patients using multiple data sources.
- Interview patients, caregivers, or providers to obtain the most accurate medication history based on patient-specific factors (eg , illness, cognition, literacy).
- Accurately use and communicate with external resources to obtain pertinent information.
- Document medication history in the EMR.
- Provide appropriate escalation and appropriate handoffs with complex or incomplete medication histories.
- Effectively communicate with other members of the health care team.
- Maintain knowledge regarding new medications and dosage forms that may be used in an outpatient/ambulatory setting.
- Monitor multiple patient queues from facilities or EMRs.
What You Will Need
Licensure / Certification / Registration:
- Pharmacy Tech credentialed from the Pharmacy Technician Certification Board (PTCB) preferred.
- Pharmacy Tech credentialed from the National Healthcareer Association preferred.
Education:
- High school diploma/GED with 2 years of experience, or Associate’s degree, or Technical degree required.
Additional Preferences
Experience:
- Minimum of two years Pharmacy Technician experience required.
- Previous retail pharmacy experience including prescription transcription duties preferred.
- Trained in general use of pharmacy computer systems
