This is a part-time position (20 hours per week). Work schedule will consist of working days/evenings and rotating weekends.

As the Medication History Pharmacy Professional, you will carry out medication histories for new medical facility admission. Under basic guidance of the medical pharmacist, you will provide complete and accurate info for each medication, e.g., drug name, dosage, route, frequency, sign, and date/time last dose was taken.

Uses available resources for medication history collection including but not restricted to: patient/family interview, medication lists, outside center medication administration records, outpatient drug store records, and doctor workplace records.

Verifies/updates client allergic reaction, negative drug response, preferred pharmacy and primary care doctor information in the electronic medical record.

Files medication histories finished in the electronic health record, sending out to the pharmacist for cosignature in a precise and prompt manner.

Files medication histories in CPW/VigiLanz in an accurate and timely way. Assists the medication reconciliation pharmacist by making sure patients identified for discharge therapy and medication reconciliation have a pharmacy services finished medication history.

Prepares education products for the medication reconciliation pharmacist as needed.

Schedule may vary according to the requirements of the facility/department and may consist of weekend, night shift, holiday, on-call, and additional hours.

Credentials

High school graduate or GED

2 years previous retail drug store experience is needed

1 years of inpatient pharmacy experience is preferred

Keeps current pharmacy professional registration or drug store intern license

National Pharmacy Tech Certification required (CPhT/PTCB)
